It is the ultimate goal of Wellness Health Care Solutions Inc. to enable independence to those with special needs. Our objective is to support those with developmental disabilities by creating services that offer personal growth, empowerment and increased self-reliance. To that end, the Community Living Assistance and Support Services (CLASS) program provided by Wellness Healthcare Solutions Inc. offers cost effective home and community based case management services as alternatives to placements outside the home.

Care Can Be Expressed in Many Ways

Funded by a Medicaid waver, the CLASS program allows the Texas Department of Human Service to provide services that are not available under the standard Medicaid program. In addition to increasing personal growth, power and self-reliance, the goals of the CLASS program achieve the following:

• Integrate people with related conditions into the community;
• Provide flexible services that reflect on the changing needs of individuals with related conditions;
• Prevent institutionalization.

Our CLASS Case Management Program is based on the premise that with education and personalized/individualized assistance, consumers can acquire the knowledge for developing skills required to manage their lives while at the same time comply with their treatment plan.  Case Managers work either face-to-face or via the telephone as a way of developing self-management. Through basic education, referral, crisis intervention, re-assessment and monitoring, the intensity and type of intervention is customized based on the consumer’s individual needs and abilities.
To facilitate emergencies or crisis situations, Case Managers are available 24 hours per day 7days a week. In addition, the staffing model includes 24-hour availability, which is dispatched and monitored through a daily call system. The auto-assignment capacity is managed through an automated call center.
